MIDDLEBURY - Charles Frederick "Chuck" Niebauer, 64, died Thursday in Elkhart General Hospital.
Survivors include his wife, Dolores "Dee" Fisher; two sons, Brook Patrick Niebauer and Adam Christopher Niebauer; and a daughter, Tammy Allen.
A memorial service will be at 12:30 p.m. on Monday in the Church of Jesus Christ of Lattter-day Saints, 3415 Pleasant Plain Ave., Elkhart.
Services will be conducted by Bishop Calvin Baker.
